Jacob Gruttadauria, RN, MSN, FNP is a compassionate and experienced Family Nurse Practitioner with a Master of Science in Nursing from Ursuline College and a Bachelor of Science in Nursing from Kent State University. He has been practicing as an NP for eight years and joined the company in December 2016, bringing a strong commitment to patient-centered care across a variety of healthcare settings.

Outside of his professional life, Jacob enjoys spending time with his family, staying active through sports and swimming, and exploring new places through travel. He and his wife have been married since 2013 and are proud parents to two children who keep them happily busy.

Jacob currently provides care at several facilities, including Lutheran Home at Concord Reserve, Rae Ann Westlake, Hopkins Rehab & Care Center, Embassy Royal Oak, The Pavilion Rehab & Nursing, Diplomat Healthcare, Middleburg Heights Assisted Living, and Welsh Home.
